Leeds City Council Services for people with dementia and Carers
The vision for Adult Social Care is to commission flexible and responsive, individualized services for people with dementia, including younger adults with dementia, and carers that are fit for purpose for both current and future populations in line with demographic changes.
The emphasis will be on services being delivered closer to home with a focus on people living independently for longer and therefore reducing over reliance on statutory service provision.
The commissioned services will be outcome focused and reflect the issues facing people with dementia and carers. All people accessing these services will be offered timely, individualized support throughout their dementia journey including, for younger adults, the transition process when they reach 65.
