Changing Lives Programme for People with Learning Disabilities
Bradford Metropolitan District Council is seeking to contract with one suitably experienced organisation to deliver an independent facilitation of person centred reviews. The organisation will deliver an annual person centred review to all people with learning disabilities in the Bradford District who have a funded support plan and will also champion best practice with regard to person centred reviews and support skills development across the wider service network. This is a very exciting and nationally innovative development, and is central to our ambition to embed person centred thinking and planning into all aspects of service and support for people with learning disabilities and their families.
